Safety rules are written in blood and we've been writting ours for a long time now. They just started.

It's not idiocracy. Most of these are in countries that either got into their industrial revolution much later than the western world or had a government that didn't care at all. We have gone through these kind of accidents too but a long time ago with far less powerful machines and learned from it. They are speedrunning the same process but with insanely powerful machines instead.

Holy shit. Seeing the nurse/hospital worker actually turn on that machine after walking right past her coworker cleaning underneath it is diabolical.
